WESTLAKE WELLNESS Client Informed Consent, Scope of Practice, and Disclaimer Agreement

Before you choose to use the services of Westlake Wellness, please read the following information FULLY AND CAREFULLY.

GOAL: Westlake Wellness is a holistic coaching service that supports clients in improving their health, emotional resilience, and quality of life. The goal is to educate, empower, and guide clients through nutritional, lifestyle, somatic, and nervous system-informed approaches to optimize wellness. This coaching work may involve support for gut health, food sensitivities, chronic pain, stress regulation, movement, and emotional well-being. Coaching requires sincere commitment, possible lifestyle changes, and a collaborative relationship. We do not diagnose or treat medical or psychological conditions and do not guarantee specific results.

SCOPE OF PRACTICE: Westlake Wellness is a coaching practice, not a licensed medical or mental health provider. We do not:

  • Diagnose, treat, or cure any disease or mental health condition.

  • Provide medical advice, prescribe medications, or conduct clinical psychotherapy.

  • Submit insurance claims or provide medical coding.

If you are under the care of a physician, therapist, or other licensed provider, it is your responsibility to inform them of your participation in this coaching program. You are strongly encouraged to consult your healthcare provider regarding any medical or mental health conditions, especially if you are taking medications or undergoing clinical treatment.

Our services include:

  • Nutritional therapy coaching, including review and interpretation of functional lab testing (GI-MAP, MRT, blood panels, etc.) for educational purposes only.

  • Use of client questionnaires (including symptom severity, medical and family history, personality assessments, and food/mood journals) as tools to guide coaching decisions. These are not used for medical or psychological diagnosis.

  • Nervous system and somatic support, including body awareness practices, polyvagal-informed education, parts work, EMDR-informed techniques, and non-clinical neurofeedback support (including use of tools such as MindLyft). These methods are not a substitute for psychotherapy or trauma treatment.

  • Personal training and movement education (provided under NASM certification) to support body awareness and resilience.

  • Mind-body education and journaling techniques for chronic pain and emotional awareness. These approaches are provided for educational purposes only and are not a replacement for mental health or medical care.

Clients may have existing diagnoses such as fibromyalgia, IBS, or chronic fatigue syndrome. Westlake Wellness does not treat or manage these diagnoses but provides educational and coaching support that may complement ongoing medical care. Any changes to diet, exercise, or health routines should be reviewed with your licensed healthcare provider.

MEDICATIONS AND SUPPLEMENTS: If you are currently taking any medications or supplements, you must notify Westlake Wellness so we can consider this in your protocol planning. You are responsible for consulting your physician or pharmacist regarding any interactions between medications and supplements. If you experience any adverse physical or emotional responses, stop the intervention and contact us and/or your healthcare provider.

LAB TESTING: Functional labs may be used to help uncover dysfunctions or imbalances. These tests are not diagnostic and are not interpreted as medical lab results. They are used solely as educational tools to guide nutritional and lifestyle coaching.

COMMUNICATION: Coaching is a dynamic and individualized process. Each person's body responds differently to changes in food, lifestyle, supplements, and movement. Adjustments may be necessary along the way. You are expected to:

  • Maintain open communication with Westlake Wellness.

  • Report any significant changes, reactions, or life events relevant to your coaching work.

  • Take responsibility for implementing recommendations, including rest, movement, nutrition, and somatic practices.

CONTACT WITH OTHER PROVIDERS: We welcome collaboration with your licensed healthcare team when appropriate. You may request that your physician or therapist contact us with questions about your coaching work.
